- Please access and complete the Required Registration Health Forms for the grade level your child will be entering in 2017-2018.
- Remember to complete the Optional Health Forms in the event your child: requires medication at school; needs to carry an asthma inhaler; haslife-threateningg allergies; and/or plans to participate or tryout for an interscholastic athletic program (at Daniel Wright only)
- Please also review the Immunization Requirements for 2017-2018 to ensure that your student has received what is required in order to start school on the first day of class (August 28, 2017).

Immunization Requirements
Evidence of immunity against the following diseases must be submitted at various grade levels: Polio, Diphtheria, Pertussis, Tetanus, Measles, Mumps, Meningicoccal, Pneumoccal, Rubella, Hepatitis B, Haemophilis Influenza Type B, and Varicella.
Failure to comply with the above requirements by the first day of the current school year will result in the student’s exclusion from school until the required health forms are presented to the school, subject to certain exceptions.
